An evaluation check list:

1. Is the purpose of the form clear?

2. Are the instructions for filling out the form clear?
3. Are the questions on the form clear?
4. Is there sufficient room for the required responses?

Test it by asking someone else to fill out a draft sample


5. If the form calls for entry of quantities, or dollar estimates, etc, is there space for the units of measurement, unit description, unit prices, calculated value and sum total?
