5. In the Main and Supplementary Content areas

• Make sure there is sufficient room for the response

  • Or direct the user to employ additional sheets if necessary
• Check boxes are a particularly useful device

  • For "Yes/No" type responses
  • But keep the boxes close to their respective descriptions
• If columns are used, make sure the headings are clearly identified with the column

  • By centering the column heading over the column
• If long rows are used in a landscape layout

  • Provide accent lines at every fifth row
